Please submit a change of address form for you or your child(ren) if your address has changed.
The Member Services department shares address updates with the PBPN Percapita office, and Providence, which distributes Minors Trust information.
Returning the form through the mail is preferred at this time. If you wish to return your form in person, you will need to contact Member Services at 785-966-3934 for department entry.
