Tribal members wishing to pick up their Per Capita payment at the Government Center need to complete the Payment Option Form and select payment option 3 and return the form 14 days prior to Per Capita Distribution.
The form is available on the Per Capita web page.
All members picking up checks will be required to show their photo ID.
