Notice to Tribal Members
All Prairie Band Potawatomi members who have not sent in an updated W-9 within in the last 3 months are required to do so by January 31, 2013. Failure to do so may cause a delay in future Per Capita payments.
Completed W-9’s can be faxed to 785-966-3917 or mailed to PBPN Government Center, Attn: Per Capita, 16281 Q Rd, Mayetta, KS 66509.
Please note that a new W-9 is required to be filled out anytime an enrolled member has a name change or address change.
