DIABETES PREVENTION LIFESTYLE COACH:
The purpose of the position is to serve as a Health Technician and
function as a member of the Diabetes Prevention Program (DPP) staff. Primary functions include assisting staff
with the preparation, implementation, and documentation of diabetes prevention
activities at the tribal site, coordinating less intensive activities,
assisting with intensive activities, and providing lifestyle coaching to
participants.

REGISTERED NURSE: Registered Nurse provides nursing care to patients in the
Health Centers and assists the Nursing Supervisor in accomplishing quality
control.

PROJECT COORDINATOR (35 HR/WK): The Project Coordinator is
responsible for managing the day-to-day operations of the “Return To A Healthy
Past” grant project. The position will maintain community garden plots and
organize activities around the production and use of traditional foods. The
Project Coordinator works with other Tribal departments and community members
to procure their support and use of materials and resources. This position
collects, maintains and disseminates data on grant activities, including
reporting as required by granting agency.

DIABETES EDUCATION NURSE/CASE MANAGER:
Provide nursing care to patients as specified in the Diabetes Grant;
primary responsibility for providing outpatient nursing care and education to
persons with diabetes who receive their care at the Prairie Band Potawatomi Health
Center as well as other duties including planning and implementing community
based diabetes prevention activities.
